South will overcall 1S if you open anything else at the 1 level, apart from that opps will be silent.

How do you bid it?

1-(1)-X(style for us is both other suits, even easier if it 100% guarantees 4 hearts)
2(GF)-2N(bal, spadestop, unlimited)
3(4)-3(cue agrees , extras)
4/(depending on cuebid style)-4
4(KC)-5(2 w/o) (or 4N-5 if that's your KC)

Now you know partner has 4 hearts, a balanced hand A, A and K, probably plus a little something else, if he has J or J10, 7 may be decent, but I think I'd settle for 6.

It just depends on how many hearts you promise with double over 1 spade. I don't think finding grand slam is realistic though.

west can essentially count 13 tricks opposite akq of trumps ace of spaces (and finesse through bidder), king of diamonds and a club singleton. yes ok there might be handling problems but partner is allowed to turn up with something extra.

since I expect opener to have at least 18 for the 4c bid I hate to say this but
what I would normally think of as a stinking spade J has me thinking easy grand

1 = 15+ nat/bal or 18+ any
... - (1) - 2 = GF; diamonds or bal, <4 spades
2 = diamond tolerance
... - 2 = nat, bal
4 = splinter
... - 4 = diamond control?
5 = yes, and a spade control with 3 key cards
... - 5 = Q ask
5NT = yes, plus a second diamond card (AK or KQ) and no K
... - 7

It is trickier if Asking Bids are not played after the splinter, for example if 4 is Last Train. In that case we probably want Opener in control, which in practise is not going to happen as often as Responder driving.

Note also that despite the auction above I am not convinced the grand is going to be great in the average I/A (club) field. South is likely to have 6 spades for an overcall of a Standard 1 opening leaving North with a singleton. So we need to draw trumps before unblocking the spades. But this also increases the danger of North having 4 or 5 trumps. Now we can ruff 2 clubs but lack an entry to finish drawing trumps. Diamonds 3-3 works but that is not exactly what we would be looking for; no doubt there are other chances too but I am too lazy to analyse it properly. In short, in an I/A field it might well be better to be in 6, since a large chunk of them are probably not in slam at all.

Finally, why do you think the 4 rebid promises 18hcp gszes? Would you not also splinter with 16-17? Maybe even with less and a void. In my world it shows heart support, enough for game and shortage. Not that the difference would stop West from driving on this hand. I think the question of how an uncontested auction would go is also interesting - presumably for those that started 1 and splintered the same but this time without any suggestion of a spade finesse.

Well only if partner doesn't have Ax/AQ, AKQx, Kxxxxx, x/J/Q, now you're in a 40% grand.

The key though if you want to get to a grand is to get W who knows about the J10 to do the asking.

(4) Queen Ask
(5) trump Queen, no spade king, but king of clubs
(6) anything else, ... promising all key cards
(7) Club singleton, Queen of diamonds

I will never reach grand, and what is the plan for 13 tricks anyway? Ruff two clubs then hook the spade? Maybe we can check for JT dub of diamonds beforehand... it feels tricky

I would suggest a sequence, but I don't really know what I would do and I'd probably embarrass myself by winding up in game very often. I don't have a club splinter after they overcall 1S, so it's going to get murky.

Grand isn't bad, say trump lead as you often do get

A
K
to A
ruff
to A
ruff (high as it's all you got)
to J

and if the weather is fine to this point you're home even if trumps are 4-1, just draw trumps with the J10 and cash out.

the auction has gone 1d (1s) 1h. obviously no reason not to play 4c as a splinter. if you have the meta agreement that you can only splinter in opps' suit after an overcall, i suggest you change that to mean only the player bidding directively after interference.
