Your options:
Play ♠A and another - this goes 2 down if E had ♠KQx a he can cash a diamond on the end, not a big worry NV at teams, you're always down, but also fails if E has H10x as he takes the K and makes you ruff the diamond promoting his 10. When you see an honour drop from W, you can't cash 3 clubs and get back to dummy to lead up to the ♠ J so you can't have it all. For that reason I'd have started cashing clubs not diamonds which I think works better in the equvalent scenario.
The alternative is to play 3 rounds of clubs without cashing ♠A.
It appears EW are 5-2/2-5 in the reds.
so if they are 3523/2254 playing on clubs before playing trumps will work, and when they stand up play ♠Ax and you will make, ditto if you just play ♠Ax
2524/3253 playing ♠Ax fails, playing 3 clubs and then ♠Ax will succeed if W started with ♠Kx/Qx
4522/1255 you can only succeed if E's stiff is the 10 and then you pick it up incidentally by playing 3 top clubs.
So I cash 3 clubs next ditching my diamond if W follows or ruffs with an honour. In the latter case he is endplayed to either take the spade finesse for me or lead a heart so I can ruff in hand and do it myself.

Maybe we should have switched to clubs to try and get a proper count in the minor suits before committing to one for the heart discard?

Last night I butchered this deal. South is declarer in 4♠ on ♥K lead. Plan the play.
I ducked ♥K but won the ♥Q continuation, East showing an even number.
I continued ♦KAQ, discarding a ♥.
West followed twice but ruffed the third round low.
I ruffed West's ♥J continuation, East discarding a ♦ (?)
What now?
